That used to happen to me all the time. And still when I end up at my appointments (since they are now more spread out) I often forget that about which I want to talk. I often think about what I want to say the night before or on my way over to the appointment. That sometimes helps. But, I always seem to have something come up that helps my therapist get a better picture of things that bother me—even if those things don’t come up often/rarely bother me, they are often things that are pretty deep rooted. A couple months ago I relived the process leading up to my one hospitalization and it was triggering for me (lots of tears).
If you are concerned that you are not covering the ground you want to cover, then you could try justthissideofsane’s suggestion: bring in a short list of things you would like to discuss. I’ve done that for psychiatrist appointments and found that it helped a lot.
